Supported Housing Services
The
Application Process
Stage
1
A
letter will be sent to the appropriate professionals and other
agencies when we anticipate a vacancy within our supported housing
service. Any applications we are holding will also be considered
at this stage.
Stage
2
The
referring agent will be contact the supported housing co-ordinator
to discuss the possible referral and to request an initial contact
form, alternatively this can be downloaded form this site. This
should be completed by the applicant and the referring agent,
and returned to the supported housing co-ordinator, who will respond
within 5 working days.
Stage
3
The
applicant and the referring agent will be invited to meet with
two of the supported housing team to complete a detailed assessment
form and risk assessment form.
Stage
4
If
the applicant meets the criteria for a place in one of our supported
houses, he or she will be invited to visit the house by appointment.
He or she will be able to view the accommodation, meet the other
tenants and the supported housing team. After this visit the decision
will be taken whether or not to offer the person a place. The
applicant and the referring agent will be informed of the decision
within 5 working days of the applicants visit.
