Community Wellbeing Services
Background and Information:
All our services provided in this section are funded by Aneurin Bevan University Health Board. We are providing these services alongside our partners in the Gwent Mental Health Consortium.

To access the service an individual:
- must live in the ABUHB footprint area
- be aged between 18 and 65 years old (consideration may be given to supporting people aged 16 and over in transition)
- be a normal resident within the ABUHB area, who is living in another health board area
and must have:
- a mental health problem
- be at risk of developing a mental health problem
- be caring for someone with a mental health problem

We have a single point of access for all the above services:
Complete the referral form and send it to our single point of access as indicated on the referral form, or directly to us, by the following methods.
By post to: Henton House, 28 Monk Street, Abergavenny, Monmouthshire, NP7 5NP.
By phone: you can call 01633 810 718 or 01873 858 275 and ask to speak to a member of staff in the Wellbeing Team who can take the referral over the phone.
By hand: drop in and return to our Henton House office.
What is Information, Advice & Assistance?
Information - prompt and accurate information to enable an individual to make an informed choice about their wellbeing.
Advice – working jointly with the individual to discuss what is important to them and what they want to achieve; considering options and reaching agreement on the way forward through a proportionate assessment.
Assistance – ensuring action is taken that will assist the individual to access appropriate care and support, or support in the case of a carer.
Examples of what this covers:
- Provide information on how the care and support system operates (including the range of preventatives services), both locally and across Wales.
- Provide information on how individuals can gain access to these services and support mechanisms.
- Provide information on how individuals can raise concerns about themselves or others who appear to have a need for care and support, or support in the case of a carer.
- Provide IAA on Housing related issues e.g. getting help at home, residential/nursing care, grants, housing adaptations, repairs, access to accommodation, prevent homelessness etc.
- Provide IAA on Financial Information e.g. benefits, direct payments, money issues etc.
- Provide IAA on Citizens’ rights, entitlements and available support.
- Provide IAA on the range of support, preventative and advocacy services available across the ABUHB area.
We can offer Information, Advice and Assistance by:

- Face to face appointments: we offer appointments in Abergavenny, Chepstow, Caldicot and Monmouth. Call us to book an appointment.
Our service offers a safe, private and confidential place so you can work with one of our counsellors on the problems and difficulties you are facing. Our counsellors can assist you to explore, make sense of the issues troubling you and offer possible alternative ways to move forward with your life where you will not be judged.
How can counselling help individuals?
We can provide counselling for individuals with a wide range of mental health and well-being issues and will support citizens with a wide range of issues. For example, counselling provision may help individuals to:
- cope with redundancy or work-related stress
- explore issues such as sexual identity
- deal with issues preventing them from achieving key goals
- cope with depression or feelings of sadness, and have a more positive outlook on life
- cope with anxiety, helping them to achieve key goals
- understand themselves better
- feel more confident
- develop a better understanding of other people's points of view
- cope with a bereavement or relationship breakdown
The service is co-ordinated by a qualified and experienced Counsellor who provides counselling and supports all the counsellors. We have student counsellors who are working towards gaining a Diploma in Counselling whilst studying in college or university. All our Counsellors are registered with a relevant professional body (e.g. BACP, UKCP) and either accredited with that body or working towards accreditation.
We can offer counselling to both men and women over the age of 18 years of age (consideration may be given to supporting people aged 16- 18 years) who live within the county of Monmouthshire or within the Aneurin Bevan University Health Board (ABUHB) footprint area.
We can also offer you counselling appointments with either a male or female counsellor.
Each of the counselling appointments last between 50 – 60 minutes. The counsellor can offer you appointments at the same time and day every week. This is our way of helping you gain the best possible outcome from the service.
This is a free of charge service.
How to access the Counselling Service:

The referral form can be returned to us via the single point of contact as indicated on the referral form.
By Phone: 01495 768840 and ask for the Community Counselling Team who will arrange an initial assessment.
All our referrals for Counselling are managed by Torfaen and Blaenau Gwent Mind; Counselling sessions will be provided by Mind Monmouthshire Counsellors from our office base in Abergavenny.
Once the initial assessment is carried out you will be placed on the list and given an idea of the waiting time to be seen for counselling.
Mind Monmouthshire is registered with and follows British Association of Counselling and Psychotherapy Guidelines.
